About Refractive Surgery
Dr. Bruce Cohen, an experienced LASIK surgeon serving St. Louis and Chesterfield, has helped patients resolve complex eye problems for years. He performs LASIK (laser-assisted in-situ keratomileusis) to surgically correct myopia (nearsightedness), astigmatism, and hyperopia (farsightedness). These procedures are performed on an outpatient basis, and are designed to reduce or eliminate the need for glasses or contact lenses in certain individuals. For more information about refractive surgery and to learn whether or not you would be a good candidate, either contact Cohen Eye Associates by email or call our office at (314) 226-1437.
Appointments
For routine office visits, Cohen Eye Associates advises that you call for an appointment three to four weeks in advance. If our patient load permits, we will work you into our schedule sooner. If you have an emergency, we will, of course, accommodate you as quickly as possible.
When you call to make your appointment, explain your vision concern. If you are having a specific problem, please describe it to the staff member on the phone so that the appropriate amount of time can be allotted for your visit.
When making an appointment with an optometrist, ophthalmologist, or LASIK surgeon at our Chesterfield or St. Louis office, please allow sufficient time in your schedule. Some appointments, such as complete exams and yearly checkups, require a minimum of one-and-a-half to two hours. This allows ample time for a thorough examination. We will ask you for a brief medical history, including any eye problems you may be having. We will also want to know about prior surgeries and about any medications you are currently taking.
Should you need to cancel an appointment, we ask that you notify us as far in advance as possible.
Non-emergency and emergency calls
If you have any problems or concerns about your vision, simply call Cohen Eye Associates anytime during business hours. Our technicians should be able to handle all your questions, including those about eye problems, contact lens refills, or prescriptions. If you need to speak to Dr. Bruce Cohen, Dr. Sharon Jick, or Dr. Kathryn Noonan, please leave your telephone number and we will contact you as soon as possible.
For prescription refills, please call the pharmacy that filled the original prescription.
If an emergency arises during office hours, call (314) 226-1437 Monday through Friday and 314-469-1684 on Saturdays.
Insurance
Our office has contracts with a variety of HMO and PPO insurance plans. Please check with your carrier regarding plan requirements, such as referral forms, authorization, co-pays and coverage for visits and specific treatments. We will be happy to file the insurance forms for the plans in which we participate. Please bring your insurance card and/or your Medicare card to your appointment, and keep us informed of any changes in your insurance coverage.
Cohen Eye Associates, Ltd. is a participating Medicare provider. This office accepts assignment for all office visits and surgical procedures covered by Medicare. We will file all Medicare claims for you, but you will be responsible for filing your Medicare supplemental insurance. Federal law requires that you are responsible for the charges not covered by Medicare as well as the Medicare Part B deductible. Please note that refraction charges are not covered by Medicare Part B. These charges must be paid at the time of service.
For after-hours emergencies, call the answering service at 314-771-6080. The answering service will contact Dr. Cohen, Dr. Jick, or a covering physician.
Payment procedures
All fees incurred during your visit or your insurance co-pay are due the day of service. We accept payment by cash, personal check, and credit card (VISA, MasterCard, and Discover).
